This form is to assist me in quoting/booking a cruise on Disney Cruise Line or Royal Caribbean Cruise Line.

Client Information

Which Cruise Line? *
There are a wide variety of stateroom choices, but let's start with the most common. Which of the below interests you? *
Ship location (Tip: If you or someone in your travel party is very prone to motion sickness, midship is best)? *
The Caribbean and Bahama itineraries for both Disney and Royal usually offer a day at their own private islands. Are you wanting to visit one of these? *
Do any of these apply to someone in your travel party? *
Travel Documents: In the quotes that I send, I will include a link to what the cruise company requires for your specific destination.  That said, it is always best to have passports for all members of your travel party.

Travel Insurance: I will inlcude Travel Insurance pricing in your quotes because I feel that strongly about the necessity of having it. Should anyone need to be medically evacuated by air from the ship, the cost to do so will be tens of thousands of dollars.  I will highlight the amount that each policy will cover for medivac services. Should you decline Travel Insurance for your cruise, I will require that in writing.
