Please print, complete and return the PERMISSION, RELEASE, and AUTHORIZATION TO SEEK MEDICAL TREATMENT form to the Parish Office. Your registration will not be complete until we have received them.

SLAM & VBS Counselor Code of Conduct

By signing up to serve as a counselor for SLAM camp or VBS I agree to the following:

1) I will be respectful of the adults who are chaperoning and leading activities.

2) I will engage and interact with the campers in my group.

3) If I am a SLAM camp counselor I will only use my phone when necessary for taking pictures of group activities or communicating with other counselors or chaperones about camp activities, otherwise I will be giving my full attention to the campers in my group and the activities we are doing. If I am a VBS counselor, I will put my phone in the box at the beginning of each day and pick it up at the end of each day.

4) I will be quiet the first time that I am asked when instructions are being given and will not talk over adult chaperones or other volunteers when they are speaking.

If I do not abide by this code of conduct, I recognize that I may not be allowed to return as a counselor next year and that the adults in charge of camp have the right to refuse to sign my service hour form.
