Adoption Application

Congratulations for wanting to adopt a pet in need. Please Note:

1) We reserve the right to perform a home visit

2) We reserve the right to approve or deny any adoption for any reason and the right to not disclose the reason

3) You must be at least 21 years of age to apply, if you live at home parent must do application

4) The application is good for 1 year from the approval date

5) We offer no guarantee on breed, health, mental disposition on any adopted pets.

6) Once adoption is complete there are NO REFUNDS for any reason. You are responsible for the new pet in its entirety.

When answer choices are offered, please check all that apply. Please be as specific as you can.

--------Altering animals ears or tail is NOT ALLOWED in any way! The pet WILL be reclaimed. The average, minimum cost of accepting a dog into our rescue is $350

--------- *Verification of fixing pup must be sent to RR by the time pup is 5 months old. *If pup/dog over 6 months old is not fixed at time of adoption the adopter must have done within 2 weeks and email RR proof.

Our adopters are now entitled to be able to participate in the New Jersey low cost spay\neuter program. 20$ at participating vets offices.

All Adoption Fees are DONATIONS being made to a 501c3 non-profit organization, are tax deductible, and non-refunable.

------------- If your dog’s expenses are covered by the adoption fee, the remainder of the fee goes to assist other dogs in our care. Your adoption contribution pays for food, medical care and daily needs. Some of the lifesaving costs your contribution covers are: Emergency transport to pull euthanasia list dogs, spaying and neutering, vaccinations, medical care, supplies and food.

-----------ADOPTION FEES ARE GENERALLY AS FOLLOWS: DOGS AND PUPPIES UNDER 1 YEAR OF AGE-$450-and up. DOGS 1-5 YEARS OF AGE-$380; DOGS OVER 5-7 YEARS OF AGE-$250; DOGS 7 OR OLDER-$150; Each animal may differ in fee from the above.

RUFF REHAB AGREES TO DO THE FOLLOWING:
............1. Give the Adopter title to, possession and control of the dog as described: For so long as the ADOPTER complies with the terms of this contract.

------------ 2. Provide ADOPTER with any and all veterinary records available for dog along with current medications.

------------ 3. RESCUER acknowledges receipt from ADOPTER of a donation as noted below to aid with reimbursement of expenses incurred to consider dog for adoption. IN RETURN, THE ADOPTER AGREES TO THE FOLLOWING:

------------ 4. ADOPTER agrees to pay a NONREFUNDABLE adoption fee, based on the fee schedule above.
----------- 5. Keep the dog as a pet and companion and not to abandon, trade, give away, or sell it, nor is the dog to be used or sold for commercial use or research purposes. If the ADOPTER no longer desires or is unable to keep the dog within the terms of this contract the dog must be returned to RESCUER. It is NEVER to be turned over to an animal shelter or passed to another owner.

------------ 6. Provide adequate, food, water, shelter, exercise, and medical care at all times and care for the dog in a devoted and humane manner for the remainder of its natural life.

------------ 7. Have the dog examined by a licensed veterinarian at least once a year, maintain appropriate vaccinations and other preventive health and dental care, and provide copies of the dog’s care records to RESCUER upon request.

------------ 8. The dog shall reside within the ADOPTER’S home, not be crated for more than 8 hours and not be left in a fenced yard while ADOPTER is not home. The dog shall NOT ride in the back of a truck or be left in a car unattended.

------------ 9. Keep the dog under the ADOPTER’S control and not let the dog roam freely outside of the home or fenced yard. Keeping the dog on a chain, rope, dog-run or similar device is not permitted. Fatal injuries have been frequently been caused due to these types of restraints.

------------ 10. Provide a humane collar OR harness for the dog to wear at all times and to keep on this collar, at minimum, contact information and the rabies tag.

------------ 11. ADOPTER must notify RESCUER IMMEDIATELY if the dog becomes lost.

------------ 12. ADOPTER must update any contact information including phone and address

------------ 13. During the dog’s lifetime RESCUER or assigned volunteer may visit the premises where the dog is kept and reclaim the dog if, in the judgment of the RESCUER, the dog is not being adequately cared for or ADOPTER has not complied with any provision of this contract.

------------ 14. ADOPTER agrees to pay all costs necessary to enforce this agreement including reasonable attorneys’ fees, costs of litigation, dog recovery fees, and all medical bills to restore the dog’s health back to adoptable status.

------------ 15. The ADOPTER authorizes any animal care or control agency to release the dog to a RUFF REHAB representative, hereby designated as Agent for the ADOPTER, if the dog is found running at large or is brought into a shelter facility for any reason.

------------ 16. ADOPTER acknowledges that the dog is adopted “as is” and that no implied or expressed warranties have been made by RESCUER in reference to the health, training and/or temperament of the dog. RESCUER shall not be held responsible for any veterinary expenses after adoption of the dog or held liable for any misrepresentations unknown to this RESCUER.

ADOPTER understands that should the dog cause personal injury or property damage after adoption, the RESCUER shall in no way be liable in whole or part for that damage and hereby assumes and agrees to hold RESCUER harmless from any and all liability for the dog, or any injury or damage that may be caused by it. ADOPTER hereby waives all claims, losses, or damages which may arise after the adoption of this dog, caused by any act or omission by RESCUER which results in personal injury or property damage to any person by this dog, and the ADOPTER agrees not to bring any legal action seeking any remedy against RESCUER in connection with this agreement or in connection with personal injuries or property damage claimed to result from any act or omission by RESCUER whether by ADOPTER or any third party.

------------ 18. ADOPTER certifies that neither ADOPTER nor anyone residing in ADOPTERS household has ever been charged with or convicted of crimes against animals or humans, including but not limited to neglect, abuse, or housing animals in unsanitary living conditions.

------------ 19. BY SIGNING BELOW ADOPTER CERTIFIES THAT THEY HAVE DONE ALL POSSIBLE RESEARCH, PREPARATION, AND UNDERSTANDS THE NEEDS OF CARING FOR A DOG. ADOPTER HAS READ A COPY OF AND UNDERSTANDS THAT EVERY ANIMAL IS AN INDIVIDUAL AND WILL NEED TO BE CARED FOR AND RESPECTED AS AN INDIVIDUAL. ALL ANIMALS NEEDS AND CHALLENGES ARE DIFFERENT. ADOPTER UNDERSTANDS AND AGREES TO ALL TERMS OF THIS CONTRACT.

Puppy Adoption Care Agreement

To ensure the best interest in the optimal healthcare for your new puppy, we at Ruff Rehab Inc. mandate the adopter read, understand and initial/sign in agreement to the statements below. These statements are strict guidelines for you the adopter to adhere to until after your new puppy has had its 4^th round of shots, which have been clearly identified in the following timeline statements:

I understand my puppy has the immunity equivalent of a newborn baby until after my puppy’s 4^th round of shots. _________

I will schedule a veterinary appointment within 48 hours of adopting my puppy. _______

After my puppy’s first vet visit, I will register within 24 hours for 30 days of free pet insurance. _______

I will register my puppy’s microchip ID online within 10 days of adopting my new puppy. ________

I will keep my puppy quarantined in a clean and sterilized area until after my puppy’s 4th round of shots. ________

I will remove shoes and wash hands when handling my puppy until after my puppy’s 4th round of shots. ________

I will not allow visitors who own other animals to touch my puppy, until after my puppy’s 4th round of shots. _________

I will keep my puppy quarantined from my own pets until after my puppy’s 4th round of shots. _______

I will not allow my new puppy outside my residence including outdoors, deck, patio, sidewalk, driveway, walks, car rides, leaving the house other then my puppy’s vet visit until after its 4^th round of shots. __________

I must immediately notify Ruff Rehab Inc. and/or Ruff Rehab’s veterinarian Animal Care Center 973-835-3733 for any sign of illness i.e. loss/change in appetite, fatigue, vomiting, abnormal stool, diarrhea, coughing. _________

I understand the healthcare of my new puppy is my sole responsibility as a good pet owner any cost incurred are not the responsibility of Ruff Rehab. ___________
