I am aware of the risks connected with Sports-Weights Training. I agree not to hold accountable or bring legal action against Emmanuel Fitness Center (EFC), their officers, agents, or employees. This waiver releases (EFC) from all liability relating to injuries that may occur on location before, during, or after activity.
By signing this agreement, I agree to hold the (EFC) entirely free from any liability, including financial responsibility for injuries incurred, regardless of the reasons or circumstances.
I acknowledge the risk involved with this activity, including, but not limited to, muscle tears, sprains, and other physical injuries. I certify that my participation is voluntary, and I have been made aware of the risks. Additionally, I do not have any conditions that may increase my likelihood of injury.
I will make every effort to obey the company personnel and all safety rules and will ask for clarification if needed.
I agree to pay for all damages to the facilities of Emmanuel Fitness caused by any negligent, reckless, or willful actions by me or my family.
I consent to have my child image and likeness (Photos and/or Videos) to be used strictly for Emmanuel Fitness social media pages and websites.
I consent to my child's participation in the activity of weights/basketball training and agree on behalf of said Minor to all of the terms and conditions of this agreement. By signing this release of Liability, I represent that I have legal authority over and custody of said participant.
