This form may be completed by the sponsoring University of Georgia department representative to assist in determining the appropriate visa category for sponsoring an international student, intern, employee or visitor.  Information on available types of visa sponsorships can be found here: Visa Comparisons

If you already know the visa category you require, you do not need to complete this form.
 
H-1B temporary Work Visa sponsorships can be requested here:
