If a UGA department or office plans to hire, or has hired, a non-U.S. citizen in a permanent position, the ISSIS office can assist with a U.S. permanent residency (aka: "green card") petition. The department will need to agree to pay all government and ISSIS fees associated with the sponsorship. These vary depending on category of filing, but generally run between $2,530. and $4,000.

This form requires information from both the international employee and the department representative, so it is best if both people are present together to complete the form.
