Thank you for using our online intake system to request a Work Visa Sponsorship for your international employee. This form is for a sponsorship in the O-1, TN or E-3 visa categories. This is NOT the form to use if you are requesting an H-1B visa sponsorship. Please contact your immigration advisor if you wish to begin an H-1B visa sponsorship.

This process is necessarily detailed and painstaking, and we appreciate your assistance. Accuracy in completing the required forms and documents will help to ensure that the sponsorship process will progress with as few delays as possible.

The workflow is as follows:

The department representative completes this form, and submits it online
The form contains links to all the template documents that we need in addition to the form (appointment letters; specific forms and letters in support of the visa paperwork, etc.) You may print out, complete, sign, and then upload these documents directly to the online form, or you may choose to send us paper copies by campus mail. It is much more efficient for us if you scan and submit your documents online.We will not process your case until ALL documents are submitted, whether online or by mail.
Upon submission of this form, your sponsored employee will receive an auto-email with a link to the secure online employee intake form. NOTE: This auto email will be sent to the email address you give us for your international employee. PLEASE BE SURE YOU ENTER THIS ADDRESS CAREFULLY AND ACCURATELY. The employee completes and submits the employee form, along with scanned and uploaded required employee document copies.
Your immigration advisor receives all required information, securely, in her email inbox.

NOTE: You may not submit a printed form to request a visa sponsorship. This form must be completed and submitted online. You may use the printed form only to collect all the required information and to draft your responses.

In order to ensure security you must create an account, with a user name and secure password. Please use the sponsoring Department name, or your name, as the user name.

The account you establish is only for this specific form. If you have another international employee for whom you need to submit a new online form, you must create a new user ID and password to access that new form for the new sponsorship request. If you re-use an old login and password, your old data will be erased, and may create backlogs and delays in the processing of that case.

IMPORTANT NOTES:

The form is divided into pages. If you complete a page and click "Next", the information on the page will be saved. If you close out of the form half-way through a page, your information on that page will not be saved.
Please DO NOT have your international employee complete this form. This form must be submitted only by the sponsoring UGA department representative. Also, do not give the login user name and password to the international employee to use for their required employee information form. Once you submit this form, the employee will receive an automatic email with a link to a separate employee form. The employee must create a separate login to access the required employee information form.
