Use this form to submit your schedule requests.
 
HOW THIS FORM WORKS
This form does not guarantee a schedule change.
  • For 2nd and 3rd grade teams - Changes are free until 10-5 @ 4pm.
  • From 8/23 @ 12.01pm onwards, schedule requests will cost :
    • Beyond 2 weeks from the request game, fee will be $50.
    • Between 1 and 2 weeks, fee will be $100.
    • Inside of 1 week fee will be a $100 forfeit as changes are almost impossible at a late time.
You will select the button appropriate to your fee and pay with your submission.
(Note, there will be a timestamp on your schedule request submission!)
From 8/23 @ 12.01pm onwards, schedule requests without payment will not be considered. 
If we are unable to make a change to your schedule, your fee will be refunded.
 
IF YOU HAVE MORE THAN ONE TEAM OR REQUEST, SUBMIT ONE FORM PER TEAM.
 
REMINDER -
NO SCHEDULE REQUESTS FOR 4th-8th GRADE AND ALL UH TEAMS DURING PLAYOFFS
