Project Access Registration Form

Project Access is a free service available to all individuals seeking housing support services. It operates along a fixed route Monday - Friday, 52 weeks per year and visits 21 agencies. This program is operated by Healthcare for the Homeless - Houston, and is supported by funding from the City of Houston. As such, all participants are required to register with the following information in order to support grant reporting compliance for HHH.

This form must be completed with the Case Manager, Social Worker, or other support person from a Community Agency. If you are an individual who wishes to register yourself, please call 713-286-6000 to schedule an appointment with the Caroline Street Clinic.
