Parties:
Parties refers to those involved in the booking of instrumental services with Musical Accord. This agreement is made solely between the couple listed on this application and Musical Accord. This form and its details are not affiliated with any third parties or representatives of the church/synonagogue or any other ceremony venue. Any information contained in this statement is subject to change if deemed necessary; For any questions or concerns regarding booking, it must be communicated directly with the musician performing for your wedding ceremony.
Use of Services:
In order to register, you are required to provide minimal information about yourself. In completing this document, you indicate that you understand and agree that it is your responsibility to provide information that is accurate and up-to-date in order to fulfill your service request/s. Should any details you provide at the time of registration later change before the date of your wedding, you must notify the musician to make adjustments to your registration form in order to fulfill your service request/s.
Fees & Procedures:
Please make a full payment via Zelle/cash or check no later than a month in advance of the requested date of service. Should you choose to make two split payments, a deposit of $100 must be presented at the time of registration as a placeholder. The deposit is non-refundable should the musician be informed of any cancellations. The remainder balance of the package must be provided to the musician again via cash/check or Zelle with the number provided to you. The remainder payment must be made 48 hours before the requested date of service.
Registration Changes: Postponing/Rescheduling/Cancellation
If for any reason you would like to cancel or opt out of services, you agree to notify the musician a month to no later than a week in advance of the requested service date; You understand the previous $100 placeholder deposit remains non-refundable. Should there be changes such as postponing the wedding and/or rescheduling the date and time of your wedding, you understand it is your duty to notify the musician to make sure of availability, clarity of service needs and requirements, and program development.
