BEFORE YOU BEGIN COMPLETING THIS FORM, PLEASE BE SURE THAT YOU HAVE OBTAINED ALL NECESSESARY INFORMATION BELOW INCLUDING WHAT INSTRUMENT(S) ON WHICH YOU WILL BE AUDITIONING. ALSO PLEASE BE VERY SURE THAT YOU PRINTER IS ONLINE AND FILLED WITH PAPER AND INK. YOU SHOULD ONLY SUBMIT YOUR APPLICATION FORM ONCE (FOR EACH INSTRUMENT).

After you click Submit, a receipt form will appear, summarizing your inputted data, along with the Endorsement Form required for auditions.
You will then print and submit the Endorsement Page to your school band director with the required signatures; be sure to enclose the appropriate audition fee.
Be sure to capitalize the first letter of proper names (John Smith, not john smith; Westwood, not westwood, etc.)

If you are auditioning on more than one instrument, complete this form once for each instrument, and submit separate signed Endorsement Forms and fees.

Be sure to check your submission for spelling, capitalization, and accuracy BEFORE you click Submit. After submitting, you will be presented with an Endorsement Page (a receipt summary of your application data), which contains the endorsement information. You will then print, obtain the required signatures, and submit that Endorsement Page to your director.
